Amazon Plan of Action: Root Cause, Corrective Actions and Prevention
How to turn an Amazon policy notice into a specific Plan of Action that connects the cause, completed corrections, controls and proof.
Start with the event chain
Before drafting, describe the sequence from normal operation to the policy issue. Identify who performed each step, what information was available, which control should have detected the problem and why that control failed.
This event chain prevents the Plan of Action from stopping at a surface statement such as human error, supplier issue or oversight. Those labels are starting points, not complete causes.
Write a root cause that can be acted on
A useful root cause is specific enough to determine a correction. For example, a listing error may trace to an unreviewed data source, unclear ownership of catalog changes or a missing approval step. The cause should be consistent with the evidence and should not assign blame without support.
If several failures contributed, rank them and explain the relationship. Do not list every possible cause simply to appear comprehensive.
Separate immediate correction from prevention
Corrective actions address the existing problem: removing affected inventory, correcting content, reconciling documents, contacting impacted customers or restricting access where appropriate. Preventive actions change the process that allowed the problem to occur.
- What was corrected and the date it was completed
- Who owns the new control and when it is triggered
- What record proves the control was performed
- How exceptions are escalated and reviewed
Attach proof with a purpose
Each attachment should support a defined statement. A training log can show completion of training; a revised checklist can show a new control; a supplier document can support provenance. Name the file and point to the relevant information instead of expecting the reviewer to infer its meaning.
Remove personal or unrelated material that the notice does not require. More pages do not compensate for a missing connection between the statement and the proof.
Edit for clarity and consistency
Use short sections that follow the case logic: issue, verified root cause, completed corrections, preventive controls and supporting evidence. Check that every date, product identifier and legal name matches the attachments.
Avoid copied promises, exaggerated certainty and filler. The best format is the one that lets a reviewer trace each claim without searching through the document.
